I actually have no idea how this clinic even got four stars my mother is going on a little over a month without any responses she has conditions that need to be taken care of by medication and the most important her dementia which they have left her a little over a month without her mental state medication that was keeping it at level and her conditions have worsened. They were called not so long ago and a urinalysis went to them and I never received a call back either I had to take her to the hospital and that's where I found out she had an infection until today with several faxes from Walgreens and even a phone call from the actual head pharmacist and emergency nurse from where her home health is from including several phone calls from me and a case actually been open and could you believe not one call yet and I still am waiting for her prescriptions to be sent to Walgreens not one phone call to say hey maybe we need to see her first or what the deal is with her medication but absolutely the only reason I have kept her here is because of Chicago clinic on Irving Park was on top of everything and had been the best for her but now I know that Rockford Oak Street health does not car

I received good service at the clinic. Received a referral for an ultrasound at Rockford Vascular. I have been calling since the week of June 22. The clinic keeps saying they have nit received the referral. When I call Oak Street I am told the referral has been sent. I am not happy.

I have been going to Oak Street health Rockford since they came to our city and have always been very happy with the care and concern I have received at this facility. I would recommend anyone of us “elder” generation to give them a try as the care is definitely centered around you as the patient. Trust me..I don’t post many reviews and can assure you if this ever becomes not the case in the future I will most certainly be the one patient that will post my experience! Thank you for taking the time to read my post and taking my thoughts into consideration when choosing your provider of service for your healthcare needs in your future.

Love my PCP and oak st. Staff! Great services,time management, easy to schedule appointments, med refills and prompt response from nurses if you have questions. Highly recommend.
